Public Referral Form
The public referral form on this website is designed for basic contact and referral information only. It is not intended for medical records or other sensitive health information. Submitting the form prepares an email using the email application on the user's device. The website itself does not store the entered form data.
Sensitive Information
If additional medical, clinical, financial, or other sensitive information is needed, NextStep may provide separate instructions for an appropriate method of communication. Do not send Social Security numbers, insurance member numbers, complete medical records, or similarly sensitive information through the public form.
Information Sharing
NextStep may share information when reasonably necessary to assist with senior-living placement or referral coordination and when authorized by the individual, family, representative, or referring party. NextStep does not sell personal information to advertisers.
Service & Compensation
NextStep does not charge families for placement assistance. NextStep may receive compensation from participating senior living communities when a referred individual moves into a community.

NextStep uses reasonable administrative, technical, and organizational measures appropriate to the information it handles. No internet or email transmission can be guaranteed to be completely secure.
